A dynamic learning experience designed for nonprofit professionals, healthcare providers, social service organizations, and community leaders who want to build a more connected, equitable, and coordinated system of care in San Joaquin County. Each session offers practical tools, collaborative strategies, and real-world insights to support whole-person care through cross-sector partnerships, data-sharing, and closed-loop referrals.

Managing Referrals, Cases & Clients October 1 | 1:00 – 2:30 PM | Zoom
This workshop is designed to help participants manage referrals and client cases more effectively within the CCN platform. Topics will include Managing Sent Referrals, Taking Action on Inbound Referrals, and Managing Cases.
Screenings November 5 | 1:00 – 2:30 PM | Zoom
This workshop will guide participants through conducting effective screenings and managing assistance requests in the CCN system. Gain practical skills to support clients with timely and accurate needs assessments.
CCN Certification December 3 | 1:00 – 2:30 PM | Zoom
his final workshop in the CCN series is your opportunity to refresh your skills and complete the CCN Certification Assessment. Perfect for those ready to demonstrate their knowledge and gain official recognition for their CCN platform proficiency.
